【原产地证书】商检局中韩原产地证书填写注意事项

1.Exporter's name, address, country:出口商 

ê只能单抬头

★需要提供实际生产商中文名称及9位组织机构代码号，如有多个生产商需和品名对应填写 

★寄件信息:

装运后7个工作日为后发证书 

2.Producer's name and address, country: 生产商

ê填写生产商的名称，地址（在已知的情况下，需提供相应生产商有效营业执照复印件备查）若生产商希望该信息保密可打：

3.Consignee's name, address, country: 进口商

ê该栏填写收货人名称、地址、国名,应填写常驻韩国的收货人详细的依法登记的名称、地址、国家。

4.Means of transport and route (as far as known)

êDeparture Date:（启运日期）            

Vessel /Flight/Train/Vehicle No. （船/飞机名航次/航班）

Port of loading : （装货港）

Port of discharge:  （卸货港，必须为韩国内）

5. Remarks

如果发票是由非缔约方经营者开具的，则应在此栏详细注明非缔约方经营者依法登记的名称和所在国家。

后发证书系统将自动生成“后发”字样（ISSUED RETROACTIVELY）

6.项目编号

不能超过20项 

7.包装上的运输唛头及编号:

ê此栏应与货物包装唛头一致，如无唛头，应填N/M字样。此栏不得出现“香港、台湾或其他国家和地区制造”、或“见提单”、“见发票”等字样。

唛头是图案形式的可采取贴唛。需要贴唛的证书

8.Number and kind of packages, description of goods

ê英文品名

中文品名

货物描述

包装数量（需与品名对应）

如货物无包装，应注明“散装（IN BULK）” “裸装（IN NUDE）”

9.HS code (Six digit code)

 ê10位编码，正本显示6位

10.Origin Criterion

ê原产地标准

（一）农、林渔牧、矿业初级产品完全国产填写为”WO”

（二）工业产品完全国产填写为“WP”。

（三）有工业品部份料件进口，符合产品特定原产地标准，填写“PSR”。

11.Gross weight, quantity (Quantity Unit) or other measures (liters, m3, etc)

ê填写每一项货物的数量、重量.

FOB金额与第7栏一一对应，并与报关单信息一致

12. Number, date of invoice And  invoicd

ê填写实际清关发票的发票号与日期，发票日期不能迟于第十三、十四栏申请签发日期和第四栏出货日期。

13．Declaration by the exporter or producer

The undersigned hereby declares that the above details and statement are correct , that all the goods were produced in
